Birth Certificate, Completed physical and shot record, Social Security Card and Proof of residency (must be a utility bill).
5)  Q What does my transfer student need from their old school for enrollment?
A
All of the above, including a current report card and test scores.
6)  Q Is there a full time nurse on site each day?
A
Yes, each school has a full time nurse. They work the same hours as teachers. (Please see individual school sites for detailed hours)
7)  Q My child takes medicine daily at lunch time. What procedure do I need to take in order for my child to receive medicine at school.
A
Nurses may administer medicine when necessary. The nurse has a permission form that the parent and doctor need to fill out, sign and return to the nurse.
