Hopewell Families and Staff -
With the very recent approval of COVID vaccines for children ages 5-11, the Virginia Department of Health is holding a special vaccination event this Tuesday from 5-7pm at Hopewell High School for students, families, and staff.
This event will offer COVID vaccinations for all ages 5 and older as well as COVID boosters and flu shots. Anyone under 18 must be accompanied by a parent. Please see the attached flyer with more details.
Please note that this clinic is optional for students and families who would like to participate. It is not mandatory.
Consent forms that can be printed and filled out in advance are attached below in both English and Spanish. Fact sheets in both English and Spanish are also attached.
