We are beginning a new weekly COVID screening testing program starting the week of October 18th. Please be sure to read the information below and complete the TWO registration/consent forms by Thursday, or as soon as possible. Once these forms are completed, new forms will no longer be needed each time you test. This optional not mandatory.
